You'd be able to accomplish that using our API's. Have you taken a look at those? You can find them at the following link:

Unfortunately that's not supported in the widget builder right now to turn the question details off; but you can make it so the tags/category selection doesn't display though. That or using the API's are our "out of box" options for the lower plans. On an enterprise plan, we would be able to create a custom widget for you on our side if that would be your preference.
